Key requirements

  • Diagnosis and proposed treatment plan from a Taiwanese medical institution, with a visa-application letter
  • Proof of funds (bank statement or a payment plan agreed with the Taiwan medical institution)
  • Return ticket or travel-agency proof
  • Passport valid at least 6 months with blank pages (plus photocopy)
  • Two white-background passport photos (45mm x 35mm) taken within 6 months
