- Passport valid at least 6 months with blank pages (plus photocopy)
- Health examination certificate issued within the past 3 months by a designated hospital
- Two white-background passport photos (45mm x 35mm) taken within 6 months
- Online application form from visawebapp.boca.gov.tw, barcoded and signed
